CPT® Code 11045 is used to report each additional 20 square centimeters (or part thereof) of subcutaneous tissue debridement performed during the same encounter. This add-on code is reported in conjunction with CPT® 11042, which covers the initial 20 square centimeters (or less) of subcutaneous tissue debridement.
Debridement involves the removal of devitalized, necrotic, contaminated, or infected subcutaneous tissue to promote wound healing and reduce the risk of infection.
Important: CPT 11045 is an add-on code and cannot be reported independently. It must be billed with CPT 11042.
Key Facts About CPT® 11045
- Service type: Surgical wound debridement
- Procedure: Debridement of subcutaneous tissue
- Reporting: Each additional 20 sq cm (or part thereof) beyond the initial service
- Code type: Add-on code (reported with CPT 11042)
- Setting: Office, wound care clinic, outpatient facility, or hospital
- Provider type: Physicians and qualified healthcare professionals
Common clinical indications:
- Chronic non-healing wounds
- Diabetic foot ulcers
- Pressure injuries (pressure ulcers)
- Venous or arterial ulcers
- Traumatic wounds with necrotic tissue
- Surgical wounds requiring debridement
When to Use CPT® 11045
CPT 11045 is appropriate when:
- Performing subcutaneous tissue debridement exceeding 20 square centimeters
- Additional wound surface area beyond the initial 20 sq cm requires debridement
- The primary debridement is reported with CPT 11042
- The procedure removes devitalized subcutaneous tissue to promote healing
Examples:
- Debriding 45 sq cm of subcutaneous tissue (report CPT 11042 and one unit of 11045)
- Treating a large diabetic foot ulcer requiring extensive debridement
- Removing necrotic subcutaneous tissue from a chronic pressure injury
- Performing additional debridement during treatment of a traumatic wound
Documentation Requirements
To support CPT 11045 billing, documentation should include:
- Patient identifiers and date of service
- Diagnosis and medical necessity for debridement
- Wound location(s) and measurements
- Total surface area debrided in square centimeters
- Tissue depth confirming subcutaneous tissue debridement
- Instruments and technique used
- Description of devitalized tissue removed
- Patient tolerance and post-procedure condition
- Provider signature and credentials
Accurate wound measurements and tissue depth documentation are essential to support the use of this add-on code.
Reimbursement and Coding Considerations
- CPT 11045 is an add-on code and must be reported with CPT 11042
- Report one unit of 11045 for each additional 20 sq cm (or part thereof) of subcutaneous tissue debrided beyond the initial 20 sq cm
- Wound measurements should reflect the total surface area debrided, not simply wound size
- Diagnosis coding should support medical necessity, such as chronic ulcers, diabetic wounds, or traumatic injuries
- Documentation must clearly identify both the depth and surface area of debridement
Always verify payer-specific wound care and debridement billing guidelines.
