Not working for you? You can reset your password over text if your practitioner has your phone number on file
You can log in for another person with their authorization - simply use the email address they have on file
If there are multiple accounts associated with one email, you will be asked which account you want to access
Either the first name or, more often, the date of birth (DOB) is not correct on file, or there is more than one account with your information on file
In either case, please contact your provider so they can correct the information - this is critical to ensure only authorized people access health information
