3. Appointment Details

  • Date(s) of service
  • CPT (Current Procedural Terminology) codes for each service provided
  • ICD-10 (diagnosis) codes linked to each service
  • Description of the services rendered
  • Provider signature (optional but helpful)

4. Payment & Charges

  • Total amount charged
  • Amount paid by the patient (if any)
  • Balance due (if applicable)
