Program fees:
Initial consultation: $125- One time. 40 minutes intake visit with nurse practitioner Fawn Workman, FNP-C, PMHNP-BC- see below for more information.

Basic Membership $250- includes visits and support from NP for weight management suppport, messaging, support tools, guidance, and if appropriate, prescription(s) sent to your pharmacy. The cost of the meds from your pharmacy are not included. This is ideal if you would like to have expert treatment and support that is personalized for you, with or without taking a medication for weight management. If you do pursue a medication option approved by the NP, you may use your insurance if it covers that particular anti-obesity medication, or you may pay cash for it (such as phentermine, QSymia, Contrave, metformin, Ozempic, Wegovy, Zepbound). Certain compounded medications may also be prescribed, which you would pay the pharmacy directly for (for instance, this is the case for sublingual semaglutide). You can meet weekly, or if busy and doing fine, monthly (or even check in by email or text some months). Don't forget we have free support groups, and offer convenient early evening hours and telehealth, too.

Semaglutide Monthly Membership: $375- That's it! All doses. Includes all the visits/ support/ and benefits of the basic membership, but also includes compounded semagltuide/ B-12 medication delivered directly to your home for self-administration (with the guidance of our staff, of course- we'll walk you through it!) We're here for you every step of the way. This is a savings of hundreds of dollars over the cost of name brand medications, and it includes so much much!

Tirzepatide Monthly Membership: Starts at $425 per month for 2.5 mg or 5 mg (price goes up from there). Includes all the visits/ support/ and benefits of the basic membership, but also includes compounded tirzepatide delivered directly to your home for self-administration (with the guidance of our staff, of course- we'll walk you through it!) We're here for you every step of the way. This is a savings of hundreds of dollars over the cost of name brand medications, and it includes so much much! Dose up to 10 mg: $575 monthly

(For compounded semaglutide and tirzepatide, there are often more than 4 weeks supply in each vial sent; you may not receive a vial every 4 weeks, depending on your med, dose, and strength of vial contents. Refills should be requested 1 week before needed. Membership fee is monthly regardless of dose.)

In an initial visit, a provider will review your intake paperwork, review or order labs (if needed), and help you with an initial personalized and flexible plan for a healthier body/ weight management.
Please email a copy of all recent labwork (in last 6-12 months) to info@hellohope.us prior to the appointment.
If you haven't had recent labwork, we may order it for you if you'd like to use your insurance. We can order a basic screening panel ahead of time when requested, to save time waiting for results.
It might be easier and less expensive to get your labs done at a walk in, stand alone lab that does direct client ordering. For instance, in Lynchburg, ARCpoint Labs on Timberlake Road has an "Annual CheckUp Panel for under $89" that has the basics, and no order or appointment is required (we encourage you to fast and drink 2 glasses of water before hand, and a urinalysis is also included). There are several Arcpoint or similar lab companies across Virginia. We may still recommend additional labs for some individuals at the appointment, however. We recommend getting a baseline body composition analysis, such as an InBody scan, at baseline and every few months during your weight loss journey. In the Lynchburg or Roanoke areas, these can be found at Complete Health nutrition stores, and the Jamerson YMCA also has one. They range $10-$20 per visit/ scan. We are not affiliated with these organizations.
We may make nutritional supplement recommendations to support your weight management, so make sure to share if you are taking any medications or supplements.
Payment in full is required for initial consultations (can be refunded or applied to a rescheduled appointment if the appointment is cancelled at least 48 hours ahead of time. No refunds for late cancellations or no shows for consultations)
There is no obligation to continue with a membership program. However, medication will only be prescribed with a commitment to follow up with Hello Hope. This is accomplished in the membership program. Payment for the first month of the membership is required prior to medication being prescribed.

Hello Hope Weight Loss Follow Up- telehealth $ 0.00

This is a request for an individual 1 on 1 weight loss follow-up appointment with the NP for members on a membership plan.

You can also join our group or drop in follow-ups to meet the minimum requirements for regular check-ins between refills.

In general, we recommend 2-3 individual or group check-ins per month initially, but if you need more or less, we are flexible. We want to offer you as much support as you need, but we know you are busy.

Don't forget you have access to messaging us in your Spruce Health portal and Body Site app.

Hello Hope Weight Loss Intake/ Consultation- telehealth $ 150.00

This initial Telehealth intake will include review of your completed forms, lab results, ordering of additional labs if needed (cost of lab work is not included and may be submitted through your insurance), an assessment by a medical provider of your unique needs and what has hindered your success in weight management, and the development of a tentative treatment plan. The NP will provide information on treatment options available to you, including nutrition/ exercise/ lifestyle orders, and then a membership program will be selected.* (If you'd like to be seen in the Forest office, which we do recommend for the first or second visit if possible, then please contact the office for availability 434-337-3646)

The appointment request must be confirmed by the office.
You must have all forms completed prior to the appointment being confirmed. You will receive an email recognizing your request for an appointment and instructions on completing forms in our secure portal. Please upload or email (to info@hellohope.us) results of any recent labs from the last year; or new labs will be ordered for you (at your cost/ self pay or through insurance).
No refunds unless canceled 48 hours or more before the appointment.

You can input your health insurance information; however, this practice is not directly in network with any insurance except some commercial PCHP and SHP plans.
- The intake and membership are paid directly by you.** You may request a superbill receipt that you can use to request reimbursement for the consultation visit using your out of network benefits, but this is not guaranteed that your insurance will do so.

* Our membership programs include evaluation and management of your weight struggles by a board certified nurse practitioner, follow-ups, education, access to tools, recipes, and support. We offer access to a convenient and secure messaging system, nutrition tracker, evening time drop in/ group appointments, scheduled individual appointments, coaching, and help as you pursue your goals. Most of our membership packages include medication shipped discretely to your door with all the supplies you need to administer safely in the comfort of your own home, and we're here to help support you! The office will review program options available to you after considering your unique needs, challenges, and if you meet the criteria to receive a prescription for medication.

Our weight loss programs vary in price (in addition to the consultation fee) and depend on the type of med or prescription, or if you are looking for support without medication.
Membership packages start at $250 per month:
$250 per month (no meds included- a prescription may be sent to your pharmacy).
$375 per month (injectable compounded semaglutide delivered to your home).
Starting at $425+ (injectable compounded tirzepatide delivered to your home).

Compounded medications are not generic versions of brand name medications. Brand Name medications containing the active ingredient semaglutide are marketed and patented as Ozempic and Wegovy. Brand name medications containing the active ingredient tirzepatide are marketed and patented Mounjaro and Zepbound.
