Before booking, there are a few quick notes....

1. MEDICAL WEIGHT LOSS:
-New patient or interested in the program?? Click "Weight Loss Consultation" to schedule with our Coordinator. This is required prior to joining the program and having a visit with the Nurse Practitioner. 
-Yes, we do have semaglutide and tirzepatide + discounts for those without insurance coverage; however, a prescription for these are between you and the Nurse Practitioner. 

2. PRACTITIONERS:
--Sabrina Goshen is not accepting new patients at this time. If you prefer this practitioner, please email admin@alignedhealthpractice.com to join their waitlist.
--Lindsey Recker is only accepting patients virtually- so be sure to click "Telehealth (Virtual) Nutrition" 
--Lisa Lemons (Nurse Practitioner) is 100% virtual at this time.

3. NUTRITION Appointments:
-New patients: request an Initial Nutrition Therapy Session (pick the VIRTUAL option if you want a video appointment)
-Existing patients: request a Follow-Up Nutrition Therapy Session (pick the VIRTUAL option if you want a video appointment)
-Just interested in testing? Select the test you are wanting and get scheduled. Feel free to select any practitioner to perform the test. 

4. Once you request an appointment, our scheduling manager will be in touch within 48 hours to confirm your time picked. At that time, you will also receive our intake packet (if new patient). 

5. CANCELLATION POLICY: It is important to note why we ask for credit card information upon requesting an appointment. Once confirmed, we save this slot for you and reserves the practitioner's time. Per our cancellation policy, if an appointment is cancelled or rescheduled less than 24 hours in advance or a no-show occurs, we do automatically charge the card on file. If cancelled or rescheduled in ample time (>24 hours), there is no fee. Your time is valuable. Our practitioner's time is valuable. We ask that you respect our time, and we promise to respect yours. 

6. WE ACCEPT INSURANCE! This will be uploaded when you get the intake packet from our scheduling manager. Your card will not be charged until insurance comes back with an EOB (Explanation of Benefits) for the amount due. Sometimes, they cover at 100%...which means nothing out of your pocket. Other times, a copay, coinsurance, or amount applied to deductible is due.

This initial session with a registered dietitian will involve a comprehensive assessment of your current medical history, medications, dietary patterns, and lifestyle habits to determine your nutritional needs and goals.

During the session, the dietitian will work with you to create a customized nutrition plan that meets your unique needs and preferences. This may include recommendations for specific foods and nutrients to incorporate into your diet, as well as tips for meal planning, portion control, and healthy eating habits.

In addition, the dietitian will provide education about the benefits of healthy eating and the impact it can have on your overall wellbeing. You will learn how to read food labels, make healthy choices when dining out, and other practical information to help you make informed decisions about your diet.

Overall, this initial medical nutrition therapy session offers a great opportunity to work with a registered dietitian to optimize your health and wellbeing through personalized nutrition counseling.

PLEASE NOTE: $40 is per 15 minutes. Initial sessions generally last 60-90 minutes, equating to $160-240 per session. If you have insurance that we participate with, we will bill them for this fee. 
During this session, our registered dietitian will review your progress since your last appointment, assess any challenges or barriers you may be facing, and provide personalized guidance and recommendations to help you achieve your health goals. Whether you're looking to manage a health condition, improve your energy levels, or optimize your athletic performance, our dietitian will work with you to develop a personalized nutrition plan that fits your unique needs and lifestyle.

The Follow-Up Medical Nutrition Therapy Session includes:

- Review of your progress since your last appointment
- Assessment of any challenges or barriers you may be facing
- Personalized guidance and recommendations for achieving your health goals
- Development of a personalized nutrition plan that fits your unique needs and lifestyle
- Ongoing support and accountability to help you stay on track and achieve your health goals

PLEASE NOTE: $35 is per 15 minutes. Follow-ups generally last 30-60 minutes, equating to $70-140 per session. If you have insurance that we participate with, we will bill them for this fee.

This is for existing patients scheduling a follow up with their nurse practitioner.

As of now, our nurse practitioners only see patients in the Medical Weight Loss program or for nutrition lab testing (ony current patients). 

**Please understand this is a "Level 3" visit; if the session is more complex (several problems addressed), the Nurse Practitioner will code as such (which increases cost)**

This session takes place at National Institute for Fitness and Sport (NIFS) in Indianapolis
Ready to start our Medical Weight Loss Program or unsure but want more info?! This session if for you! Meet with our Weight Management Coordinator.

During this 60 minute session, you will receive an InBody scan that measure body fat mass, muscle mass, body water, weight and more. After the scan, the Coordinator will discuss your specific goals, what our program includes, and how it could help with goals. Cost, insurance coverage, and scheduling with the Dietitians, Nurse Practitioner, and Exercise Specialists will also be done at this time (if you and the Weight Management Coordinator decide we are a good fit)!
