IMPORTANT NOTICE: This online booking serves as an APPOINTMENT REQUEST. Our office will review your request and, if approved, send an APPOINTMENT CONFIRMATION & NEW PATIENT FORMS to the email address provided.
CC PAYMENT & SECURITY NOTICE: A credit card on file is required to book & hold your appointment with the Acupuncture Physician, but you will not be required to pay until your arrival and may use whatever payment method you desire. Credit card information is saved on a secure HIPAA compliant merchant processing server and is not viewable by any office personnel.
CANCELLATION/NO SHOW POLICY: eMDe Cares reserves the right to charge the credit card provided for the full appointment amount if you NO SHOW without notifying our office of cancellation at least 24-hours before your appointment. We appreciate your understanding that Practitioners & Staff are deeply committed to serving your needs at the designated time but may suffer avoidable financial loss when appointments are not kept.
