Book this appointment if you are a new patient who would like to be seen in our clinic ?for a comprehensive sleep health consultation.

Book this appointment if you are a new patient who would like to be seen via telehealth for a comprehensive sleep health consultation. Requires a stable internet connection and a computer, tablet or cell phone with a working microphone and camera.

Book this appointment if you are an established patient and would like to come into our clinic for sleep apnea device follow-up. 

Appointment Notes: 
  • Please make sure we can access your most recent download data (remotely) or bring your download card with you to your visit. 
  • If you think you might need hands-on troubleshooting, bring your mask, machine, and all its parts.  
  • If you need BMV paperwork completed during your visit, bring a printed copy with you and let us know. There is no additional charge for BMV paperwork completed during an office visit. There is a $45 administrative fee for BMV paperwork requests received >30 days after the most recent qualifying visit.

Book this appointment if you are an established patient and would like virtual sleep apnea device follow-up. 

Virtual Visit Requirements: 
  • You must have a stable internet connection and a computer, tablet or cell phone with a working microphone and camera. 
  • You must make sure we can access your most recent download data remotely, either through your modem and machine app or through Sleep HQ with a self-download. 
  • If you think you might need hands-on troubleshooting, have your mask and machine assembled and ready for our session. 
  • If you need BMV paperwork completed during your visit, let us know. There is no additional charge for BMV paperwork completed during an office visit. There is a $45 administrative fee for BMV paperwork requests received >30 days after the most recent qualifying visit.
