I curranty accept NH Medicaid and Medicare and am in network with Anthem Blue Cross Blue Shield, Tufts, United, Aetna, Tricare, Harvard Pilgrim, and CIGNA.
Medicaid appointments are limited to 2 per year, then billing will be cash pay/counseling.

 

This visit includes a comprehensive health history review, full physical with breast and pelvic exam as indicated, review and collection of labs including blood draw or pap smear if needed as well as health counseling based on current health needs. If you are new to the practice and have a health concern to discuss, we can complete that with this appointment as long as you have not had an annual in the last 12 months elsewhere. 
If you are up to date on your gyn care, but have health care needs, please book the "Establish Care" appointment. 

Topics may include 
Well Women's: contraception and family planning, health screenings, health goals, medical care for gyn health care needs.
Peri/menopause: discomforts of perimenopause such as hot flashes, insomnia, mood changes, vaginal dryness, pain with sex, hormone replacement therapies, health screenings health goals.
This service is routine care that most insurances cover. 
This appointment typically requires in person care. 
This visit includes a comprehensive health history review, full physical with breast and pelvic exam as indicated, review and collection of labs including pap smear if needed as well as health counseling based on current health needs. If you need support with new symptoms, and are established in care, but have not been seen for a year, you will chose this appointment for full review of care needs.

Topics may include 
Well Women's: contraception and family planning, health screenings, health goals, medical care for gyn health care needs.
Peri/menopause: discomforts of perimenopause such as hot flashes, insomnia, mood changes, vaginal dryness, pain with sex, hormone replacement therapies, health screenings health goals.
These appointments are reserved for women who have been seen at H&H in the last 12 months, but have additional care needs or follow-up needs. 
Topics may include: urinary tract infection, medication adjustments or follow up, STD testing, vaginal odor or itching, breast changes, mastitis, other gyn problem topics that require follow-up. Referral may be needed. If in doubt, ask your PCP for a referral.
Appointments may be available as virtual upon request
During these appointments your care is addressed with the lens of mind, body and soul. When we have life transitions, one can't shift without others shifting around you, either expectations of your own, parts of your own life, or boundaries and structures for those around you to meet you where you are in your journey. I will be your mirror and sounding board, this will be a space for ideas, inspiration and accountability. 
These visits are reserved for times where you want further support with care that may be outside of the a non insurance billable service (cause you know, insurance does not cover these types of holistic life care needs). 
Your initial appointment needs pre-approval with Sarah prior to booking. 
These visits may include follow up care for symptoms of concern, work on learning about moods and symptoms in the menstrual cycle, work on boundaries, self care, nutrition and supplement support or other non medical counseling services. 
The initial appointment is $125, and follow up appointments are $110, or 3/$300 or 4/$400 when a multi-appointment commitment is made and pre-paid.

Thinking about pregnancy? This visit is an opportunity to review health history and medical concerns related to planning your pregnancy. Topics may include fertility awareness, readiness for pregnancy, and genetic risks, supplements and lifestyle adjustments for a healthy you. We can also review birth care options in the region such as hospital, birth center vs home birth providers. 

All things contraception! During this visit we will review the full spectrum of contraception options from fertility charting/natural family planning, to hormonal options such as the pill, patch or ring or hormonal and non hormonal IUDs and implants. We can order IUD devices during this appointment to insert at a follow up appointment based on insurance coverage.
In Office or virutal opon requst for return patients 
This appointment is if you are not due for an annual exam, but want to discuss contraception.
While H&H no longer provides birth related care, you may choose to use consultation like services for your pregnancy. During these appointments you will have a basic assessment with fetal heat tone checks, maternal vitals, review birth location options, birth plans, pregnancy and birth advocacy support, review of newborn care services and as needed labs (such as confirmation of pregnancy support or genetic testing with Quest).
These appointments are reserved for IUD insertion, follow up care and removal. Must be an enrolled patient and dup to date on your annual exam. 
Care in person only. If you want a consultation for ordering your IUD, please use either the Annual Exam appt or Establish Care appt.
Gender reveal blood draw - This test can be collected as early as 8-9 weeks, ideally once the pregnancy has been confirmed by ultrasound or doppler. The sample is mailed the day of the blood draw (for appt prior to 3pm), and results will be posted the following day. Using a small sample of mom’s blood, SneakPeek tests for the presence of male DNA (Y-chromosomes). If no Y-chromosomes are found, mom is carrying a girl.

There is no requirement to be a client of mine for this test.

This visit is for young ladies under the age of 18 who want to establish care. 
The visit will entail a comprehensive review of her medical history, and an exam where appropriate based on care needs (usually no pelvic exam at this age). During this visit we can review the menstrual cycle (physical and emotional), sex ed or symptom treatments based on the needs at the time. 
Common reasons to book this appointment are heavy periods, irregular periods, need for contraception, STD testing, painful sex/urination. 
Referral may be needed, if in doubt, ask your PCP for a referral.
This visit is for established patients to H&H who are less than 18 years of age who need follow up care or problem-related care. This may include medication follow up, STD testing, contraception changes, vaginal or hormonal symptom support. 
Referral may be needed, if in doubt, ask your PCP for a referral.
This appointment may be available as a virtual appointment by request.

For young women who want more long term support on topics related to their health. Examples would be support with moods in the menstrual cycle, eating and supplements for health, one on one sex education, all things periods (pads, tappoons oh my), and use of the Feelings Wheel in Action, learning about and creating boundaries with the self and those around you. 
These appointments are non medical care, but all the support that may be be lacking in the school or pediatric medical model. 
Reserved for girls who are showing signs of puberty or who are already mensurating. 
These appointments can take place in person, or by virtual platform. 
In person group sessions can be arranged by email.
For young women who want more long term support on topics related to their health. Virtual appointments available on request. 
This appointment is for women who need to establish care, but are not due for an annual gyn exam. If you are due for an annual exam, please select that option instead. 
Examples include new patients with plans for care related to hormone replacement, contraception care, postpartum depression/anxiety. Essentially for all women's health care for women who are up-to-date on their annual physical exam or for women who want to set up care for a second opinion, or to just become established with Hearts and Hand's with plans for future care needs. 
Virtual care can be arranged with approval, a referral is recommended with your PCP, but not required to set up the care.
