IMPORTANT BOOKING INFORMATION:
Cuozzo Health offers integrative medicine services for adult patients 18+.
A $50 deposit, credited toward the cost of your visit, is required for all NEW PATIENT bookings, or any therapeutic services booked via the online portal (n/a for Discovery Call, Follow-Ups).
If unable to keep an appointment, cancel/reschedule at least 48 hours in advance to avoid a $50 cancellation fee (charged to the card on file if no deposit was paid).
If you cancel with at least 48 hours advance notice, but cannot reschedule, the deposit will be refunded within 14 days.
The booking policy is explained on all booking/reminder emails, and consent forms.
Direct Patient Care (DPC) pricing is a self-pay model. We accept cash, major credit cards, HSA/FSA for payment of services (no checks or app payments). Payment is due at time of service.
The visit cost does not include costs for lab testing or supplements, unless otherwise stated.
You may submit a superbill to your insurance for possible reimbursement of all services and lab testing.
The consent and intake forms MUST be signed at least 72 hours prior to the first appointment, otherwise your service will be rescheduled (the above deposit policy will apply).

THIS IS YOUR FIRST STEP ON YOUR HEALING JOURNEY TO WELLNESS!

Initiating integrative care includes a 75-minute Initial Consult, required for all new IM patients, or those who have not had an IM visit within the past 12 months. Price includes a 15-minute LAB REVIEW phone call, once all lab results are back, to discuss the results and next steps. If we did not need to order labs during the Initial Consult, we will use this as a follow-up call to check progress.

Follow-up consultations are offered for established patients who have already had an Initial IM Consult and have been seen within the past 12 months.

This VIRTUAL visit includes:

30 minutes of face-to-face time with Cheryl
Discussion about how your wellness journey is going and any concerns or new issues
Follow up of your previous treatment plan and labs
Tweaking of your care plan in order to remove barriers to wellness
Further lab testing if recommended
Cheryl's time spent outside of your visit (up to 2 hours) reviewing, researching, and making evidence-based recommendations

Please provide:

A list of all the medications and supplements you are currently taking

Auricular Acupuncture 40 Min $ 50.00

This ear acupuncture and acupressure treatment involves gentle placement of up to 5 needles or seeds and beads to specific sites on each ear. The recipient then sits and relaxes quietly. Those receiving acupuncture find a sense of empowerment; that their body has the power to heal itself. Scared of needles? No worries! Tiny metal beads or seeds can be used on the same points as a form of acupressure. The beads and seeds can be left in place for up to one week. They give the same result at a slower pace.

Cupping Therapy 40 Min $ 65.00

In cupping therapy, special cups are placed on the skin and air is pulled out to create a vacuum. This helps to aid circulation and lymphatic flow, which may help relieve muscle tightness or stiffness; reduce pain or swelling, or improve athletic recovery. Many patients find it to be the most relaxing and enjoyable part of their visit, and often fall asleep! 1-2 body parts may be treated per session.

Open to all adult patients. This service is held at the Berlin CT office.

15-minute Lab Review Phone Call (AFTER INITIAL CONSULT) 15 Min $ 0.00

This is a one-time booking included in the price of the Initial Consult, so that we can review lab results once they are all available. This 15-minute call should be booked 10-14 days from the day you have your labs collected. We will wait until all results are available to have this call, unless there is a critically abnormal result that needs to be addressed immediately. If we did not need to order labs during the Initial Consult, we will use this as a follow-up call to check progress.

It is the patient's responsibility to ensure that this call is scheduled.
Cheryl will attempt to reach you by phone once, at the time you schedule, at the primary phone number in your patient profile. Please note that the call may come from a 'private' or 'unknown' number. If there is an alternative number you would like us to call, please message us in the patient portal at least 24 hours prior to your appointment. *If there is no answer, the session will be need to be rescheduled at a rate of $50.00.

15-minute Phone Check-In (PACKAGE ONLY) 15 Min $ 0.00

This booking is for established patients who are enrolled in a Package Plan. Schedule your 15-minute phone check-in with Cheryl, if included in your package. It is a great opportunity to review your care plan in between regular follow up visits, or briefly discuss progress or concerns.

Cheryl will attempt to reach you by phone once, at the time you schedule, at the primary phone number in your patient profile. Please note that the call may come from a 'private' or 'unknown' number. If there is an alternative number you would like us to call, please message us in the patient portal at least 24 hours prior to your appointment. *If there is no answer, the session will be considered fulfilled for the purpose of tracking package sessions and cannot be rescheduled.
