Once you've entered some basic information, you'll receive e-mail confirmation. If you don't see it, please check spam, it contains important details regarding your appointment.
Privacy. Your information will be used solely for communicating about upcoming appointments, coordinating treatment, and insurance repayment. Our secure platform, OptiMantra, ensures maximum security for your personal and health information.
Rescheduling/Cancelation. Please ensure cancelation/ rescheduling requests be made in advance by 24+ (or more) hrs; as less than 24 hours is often not enough time to replace the appointment, any cancelation request after that time is subject to charge the full rate. Possible exceptions include unsafe driving conditions, illness, and/or other emergency.
Payment is collected after the visit using Card, VenMo, Cash, or Check.
Appointments include physical and/or lifestyle medicine, depending on patient preference, need, and goal. Your program may include education and/or coaching in nutrition, mindful movement, bodywork, creation of Self process, and/or other holistic therapies.
If you plan on using insurance, please bring your member ID.
We will bill your insurance provided directly for services.
Our appointments are billed on a suggested sliding scale (avg. $149-$229). This means you will decide on your rate based on the income-based information we share. Income verification not required. Those who can afford to pay more gives us the flexibility to work with those who need our services but cannot afford them.

Whether you are pursuing a health goal, or resolving a long-standing concern, sustained progress normally requires a strategic approach over a long period of time. While the time needed depends on the patient, condition(s), and resources, it's common to meet weekly until desired results are sustained for 2-3 months, then to spread out sessions to twice/monthly, monthly, then once/season or as needed for additional healing/health concerns/learning goals.
