$200/Month for the first 3 months. The initial appointment with nurse practitioner to review patient's medical history, weight loss goals, and treatment options. Covers cost of labs needed for safety or metabolic concerns (does not include cost of labs for hormones or wellness). Followed by 30-minute appointments every two weeks with the nurse practitioner to review diet, exercise, medication guidance, and medication refills (bi-monthly appointments are Telemed for convenience).
$50 Deposit at time of booking, deposit goes towards appointment cost, no refunds for no show$200/Month for the first 3 months. Labs needed for diagnosis can be billed to insurance, or we offer affordable lab packages. After the first 3 months, the membership will decrease to the wellness membership $50/month.
$50 Deposit at time of booking, balance goes towards appointment cost, no refunds for no show$120/visit for patients that only want to address a single need like an acute illness. Visit does not cover the cost of medications, labs or images (xrays, CTscans). Labs and imaging can be billed to insurance, or we offer affordable prices for labs and imagining.
