Fee and Cancellation Policy
We require a $100 deposit to hold your appointment for any Functional Medicine, Sexual Wellness, Hormone or Weight Loss visit.
 50% of the service charge will be required for O or P-Shot.
 

No call, no show, cancellations within 24 hours with NP; $100 per hour scheduled or deposit is kept. 
No call, no show RN. $50 will be required prior to visit.
If you reschedule after a no show, full payment is required prior to appointment.
Weight loss - 15 min. consult 15 Min $ 50.00
Discussion (phone or telehealth) of weight loss options.  Health screening questionnaire and past medical history forms are required to be completed before visit. 
Weight loss options include: 
  • Peptides: AOD 9604, Semaglutide, MOTs-C, Kisspeptin and combinations.
  • Medication/Vitamins: HCG, Fat burning and/or metabolism boosting options, Lipotropic injections, B12, and other combinations to be discussed.
  • Lab orders may be necessary (A1C, lipids, CMP, CBC, ect). If you have labs, please have them available.
Next steps: NP review of past medical history, labs, and individualized plan.  New weight loss patient visit with NP required. (Telehealth or in-person).

Peptide Therapy - 15 min. Consult 15 Min $ 50.00

Telehealth, phone or in person visit is to review if you may be a good candidate for: Semaglutide, AOD, BPC-157, CJC/IPAMORELINE,  Kisspeptin, Epithalon, GHK-CU, Melanotan II (Tanning), MOTs-C, or other peptide related to a health concern or wellness. 


Note: All peptide therapy MUST include past medical history, current medications, and consult to review need and use.  Peptide therapy may require an exam and lab work. Peptides are all prescription medications prescribed to those with the intended use of improving health and/or with in conjunction with other treatment plans recommended by the treating health practitioner.   

HCG Diet - 15 min consult 15 Min $ 0.00
Telehealth or ?phone conversation to discuss HCG diet.
Weight Loss - New Patient 30 Min $ 200.00
During this visit, the health practitioner will:
  • Review past and current lifestyle (diet, exercise, stress, etc) and weight loss goals.
  • Review of weight loss medication and/or peptide options.
  • Discussion of current medications, hormones/hormone therapy, health and weight goals.
  • Ordering of labs to review blood work related to ability to loss weight, blood glucose, and general lab panels related to health status.
* This visit is limited to weight loss and hormones related to weight loss.  Root cause (functional medicine visit with additional intake forms are required to discuss other health conditions)
Peptide Therapy - Refill + Follow-up 30 Min $ 125.00
This visit may consist of reviewing lab results to continue peptide therapy, ordering of medication refills, adjusting medication, discussing treatment plan.
Vitamin Injection(s) 15 Min $ 25.00
Please fill out our vitamin intake form so that we may be able to direct you to the correct vitamin or amino acids which may be best for you.

Advanced lab testing is available to determine that exact vitamin, amino acids, essential fatty acids you may be lacking.  The lab test will help determine the amount and daily dose required to get you back to feeling your best.
RX refill request 20 Min $ 50.00
Telehealth, in person or phone conversation to discuss medications to refill. Labs may be required for refill.

If you are not a current patient. You will be required to complete health intake forms, labs for all weight loss and hormone medications, and telehealth or in person visit by the NP.
Functional Medicine - New Patient 70 Min $ 300.00
In depth consultation and discussion of history from childhood through present to help determine root cause of current or chronic health condition(s).   Review of past labs and notes from primary care providers, and specialists.   Ordering of general and specialty labs.

  • Diet and nutrition counseling and plan.
  • Discussion can include: weight loss, peptides, medication options, review of supplements, food and environment allergies, toxins, GI, thyroid, metabolism, mood, sleep, or other current health conditions. 
Functional Medicine Visit - Follow Up 40 Min $ 200.00
This visit can be in person or telehealth.
  • Review of health concerns
  • Review of prescription medication, including hormones, weight loss and peptide therapies.
  • Review your lab results (blood, GI, food allergy and/or other lab specialty results received).
    • A treatment plan, including diet, supplements, lifestyle, medications, and peptides will be individually developed and discussed.
  • If specialty labs are to be reviewed, each lab interpretation* will be $100 + exam cost.

* Lab interpretation costs include: research of each lab, interpretation of each lab, treatment plan developed for any health concerns, review of diet, exercise and supplements recommended, customization of compounding formulas or peptides, calling in orders and prescriptions, and setting up diet and supplement recommendations into a portal that you have access to.  Many times, our health practitioners will spend 2-3 hours with these processes before and after we see you. Every treatment plan is individualized.
Lab Results Review 40 Min $ 150.00
In person or telehealth.
  • Review of advanced lab panels (GI, food allergy, metabolism, genetics, cortisol and specialty hormones).
  • Individualized treatment plan, including diet, supplements, lifestyle, medications, and peptides will be discussed.
  • Each specialty lab interpretation* will be $125.
  • Review of blood labs included with visit

Pediatric Functional Medicine Visit - New Patient 60 Min $ 250.00
Pediatric visit that may help determine the cause of ADHD, Autism, stomach pains, GI, mood, and growth concerns, diet deficiencies.
Testing recommended for any of the above health conditions include: GI stool, food intolerance, and metabolism testing (vitamin, minerals, fatty acids, amino acids, yeast/candida, digestive enzymes and toxins), and methylation (MTHFR). 
 *Treatment plans are individualized based upon health concern and test results.

Labs required.  If you have lab results from your PCP from the last 90 days, please bring them.  In house discounted labs available.
Hormone/Peptide Therapy - Follow Up 30 Min $ 150.00
Review of lab results and review of treatment plan, which can include hormone and peptide therapy, diet, supplements, and lifestyle.
Sexual Wellness - New Patient 40 Min $ 200.00
Review past medical history, sexual wellness concerns. Review treatment options, such as: hormone replacement, medications, creams and peptides.  Ordering ?of labs ?may be required.
O-Shot - Women's Health 90 Min $ 1,200.00
