INSTRUCTIONS:
Step 1: Scroll down, make your visit selection, and click "Proceed"
Step 2: Select your preferred visit time
Step 3:
- Enter patient information if you are new. Existing patients with a current email address may access a verification code.
- In the "Reason For Visit Or Comments" box at the bottom of the page:
- Please list a reason for your appointment (back pain, menopause, etc)
- Please list insurance company (Regence, etc).
- Please list good days and times for your appointments (ie: Tues mornings, or Tues between 10am and 1pm)
- Feel free to ask any other questions in this box
I will contact you by phone within 1 week to schedule/confirm appointments. Generally, new patient call backs are on Mondays. If I hold a spot for you, I will hold it for 24 hours before releasing the time slot.
