Thank you for choosing Purposeful Pediatric Mobile Medical Service to care for your child today. We believe your child's health is a top priority and getting them back to their healthiest state is our goal! We partner with caregivers to manage common childhood illness and infections, as well as providing sports physicals at your home and at your convenience starting at 6 months old! We bring all necessary equipment and testing supplies needed to diagnose, treat and manage your child and if needed, we have access to send samples to LabCorp for further testing... trust me, we have got you covered!
Please book the appropriate appointment and complete the intake information. Appointments are confirmed with a text and/or a telephone call.

ALL PETS must be put away during the visit.

Service Charges occur once the Provider arrives to the home.

Cancellations REQUIRE 2 HOUR NOTICE prior to the appointment time to avoid the $75 Late Cancellation fee.

NO SHOW FEE OF THE FULL VISIT PRICE is applied when the guardian or authorized adult (Over 18 years old) is not present at the scheduled place of service within 10 minutes of the scheduled visit time.

You are responsible for FULL PAYMENT SERVICES if we do not file your insurance
You can receive an itemized invoice to file with your insurance for reimbursement if you choose
