Please plan on spending 20-45 minutes on new patient paperwork.  It will be included in your new patient email.  If you need help or need a paper copy, please reach out to me.  

If after an initial consultation and you would like to enroll in a membership, I am happy to apply the cost of the consultation to a membership plan. 

There is a $50 nonrefundable cancelation fee if cancelation is less than 48 hours.  If you do not cancel and are a no show, you will be charged the price of the appointment.  I understand emergencies come up, please message me if this is the case and we can discuss.  

If we are meeting virtually you must be ready to go at the time of your appointment.  I will wait for 10 minutes.  For all telephone encounters I will attempt to reach you once at our appointment time.

Reach out to me if you would like to do monthly payments towards a membership.  There is a small price difference to cover processing fees. 

*Packages and maintenance memberships are available for purchase after your initial consultation.


The cost of the visit does not include the cost lab testing or supplements.  However, you may submit a super bill to your insurance for possible reimbursement if you have out-of-network reimbursement. I do offer a discount on all supplements through Fullscripts or Standard Process. 

I am currently unable to take Medicare patients at this time. Please message me if you would like to be put on my wait list, I anticipate changing my status with Medicare early 2024.  
During this initial visit, we will get to know each other, review your health history, goals, lifestyle habits and patterns, and more. At the end of this visit, we will have the start an initial plan for you. In the next few days you will receive a personalized care plan detailing what we have discussed for the next steps. This visit is much longer than a typical new patient office visit. I  also spend a remarkable amount of time prior to meeting you and after our visit, reviewing your history, personal goals, visit notes, and lab results to carefully formulate a plan of care using an evidenced-based approach.

This visit can also be to simply get a “second opinion” about care provided elsewhere!

This visit is required for all new patients and for those I haven't seen in the last year. Please have the following information available and submitted in the patient portal prior if possible:
  • All intake paper work (plan to spend about 30-45 minutes on this) included in your new patient email
  • Copies of any recent lab results
  • Copies of any other pertinent medical records you would like me to review 
    • The last year, at least, including your most recent annual exam from your primary care provider
    • Any provider visits that relate to your main concerns

You may submit a superbill to your insurance for possible reimbursement.
This six-month membership saves you money and gives us a solid amount of time to get to know one another with 8 visits over the six months with unlimited messaging.  This membership allows us to take a deep dive into your health and begin to heal and restore your body, mind, and spirit, bringing you back to a state of wellness.

Your 6-month Healing Journey membership includes:
(1) 75-90minute Initial Consultation (first visit)  
(2) 45-minute Follow-ups  
(2) 30-minute Follow-ups  
(3) 15-minute Phone encounter (between follow-ups)

THE NEXT STEPS ON YOUR HEALING JOURNEY TO WELLNESS! Follow-up appointments are offered for established patients once you have had your initial consultation. At this visit we will check in, talk about how your wellness journey is going, discuss any concerns and tweak your 'road map' in order to remove barriers to wellness. This visit is much longer than most conventional office visits. Jamie also spends a significant amount of time before and after, reviewing your case and updating your plan of care using an evidenced-based approach. It is recommended to have a follow-up visit at least once per month, or more frequently, depending on need.

For established patients to have a 15-minute phone call with Jamie. Receive advice about a non-emergent health concern (not currently part of your care plan) that may pop up in between your regular integrative care visits or for simple questions, concerns regarding current therapy.

Jamie will attempt to reach you by phone at the time you schedule, at the primary phone number listed in your patient account. If there is an alternative number you would like us to call, please message Jamie in the patient portal at least 12 hours prior to your appointment.  Please note that the call may come from a 'private' or 'unknown' number. The card on file will be charged for the full duration of the appointment if there is no answer.
