Upon receipt of the invite to the patient portal, please complete your intake documents and upload a copy or picture of the front and back of your insurance card in the document section. Upon insurance eligibility verification you will receive confirmation of your appointment . Please have these steps completed within 48 hours after receipt of this email to prevent denial of your appointment request which will delay receiving the care that you are seeking. We will not hold an unconfirmed appointment for more than 2 days.

This is a cash-only service. No other service can use this appointment slot. The initial appointment is conducted virtually to obtain medical history and to order any necessary labs prior to medication initiation. This appointment requires a non-refundable 25 dollars deposit to make your appointment. The 25 dollars will be deducted from your next visit. All questions will be answered during your initial virtual visit.
