Welcome! Reserve your New Patient 90-minute visit. As you may know, our integrative holistic medicine practice addresses possible factors that cause disease, by focusing on creating or restoring health. You are unique and this integrative functional medicine approach necessitates collection of detailed information that you may not have ever provided in the past, but is essential to the success of this approach. It is important that you fill in the intake form as completely as possible. It is lengthy but it will save us a lot of time, allowing me to help you better. Please feel free to fill in only what you deem necessary. For us to receive and review the information, please try to submit the forms at least 2 days prior to your appointment. After your encounter with Dr. Johanna will put all your information together and generate a report with the assessment and plan, along with the recommendations for testing and handouts for your visit.
