Medically Supervised Weight Loss Program. Schedule a consultation and speak with our Nurse Practitioner Rita about your weight-loss goals.
Rita will discuss what is required to participate in the program including medical clearance, labs, diet, exercise, medication options, and medication side effects. The plan will be individualized to meet the needs of the client so that you can successfully reach your weight-loss goals.
You can choose an office visit (local resident) or a virtual visit. Once confirmed, you will receive a reminder email with consents and questionnaires. Once payment is received, You will receive an appoitment confirmation for office visit or link for virtual visit.
MEDICAL CLEARANCE IS REQUIRED BEFORE STARTING THE PROGRAM
RI-VIVE Wellness requires a non-refundable deposit to be made for your appointment. This fee is used towards your services and protects your individualized wellness appointment. Please review in detail our cancellation policy.
Mobile DOT exam including required Urinalysis (not urine drug screen) for Solano County only.
If you need services outside this area there is an additional fee of $1.00 per mile.
