This appointment is for anyone who has not been seen under the new Root Cause Clinic. Any patient that has seen Dr. Austin or Keith prior to Root Cause will still need to book this appointment for your first visit. A $75.00 deposit will be collected prior to the appointment and will be creditited to your account. This is non-refundable if appointment is not canceled within 24 hours of appointment.

This visit is to discuss chief complaints that do not require routine follow up. This is not for chronic disease managment.
