This is only for rechecking labs for current testosterone replacement patients.

Men's Testosterone Replacement Therapy (New Patient) 45 Min $ 199.00

Initial visit consultation for Men's Health. This includes labs that check hormone levels, thyroid panel, PSA, vitamin D, and cholesterol panel.

A (non-refundable) deposit of $50.00 will be charged. The deposit will go directly towards your consultation fee.

Women's Hormone Replacement Therapy (New Patient) 45 Min $ 0.00

Initial visit consultation for Women's Health. This includes labs that check hormone levels, thyroid panel, PSA, vitamin D, and cholesterol panel.

A deposit of $50.00 will be charged (Non-refundable). The deposit fee goes directly towards your consultation fee.

Department of Transportation (DOT) FMCSA exam 30 Min $ 100.00

A complete physical exam to assess the ability of the driver to safely drive. The exam includes a full physical screening (vision, hearing, blood pressure, physical, reflexes, urinalysis). The exam is performed by a FMCSA certified provider.

There is a $50.00 (Non-refundable) deposit. The deposit will be applied to the cost of the DOT exam.

The DOT exam is $100.00.

Weight loss (In person) 30 Min $ 85.00

This is for a weight loss consultation in person. The consultation fee is $85. Labs are available at an additional cost.

The medications that will be discussed during this visit include: Tirzepatide, Semaglutide, Adipex (Phentermine), and Contrave.

The consultation fee does not include the medication. The medication will be pain for at the pharmacy.

The $50 deposit will go towards you consultation fee.
