When booking for a NEW office visit appointment (whether in office or telehealth) you will be sent an email, from our office, notifying you of intake forms that are required for the appointment.
We must receive these completed/submitted forms 72 hours before your appointment, if we do not receive the forms your appointment will be cancelled. There will be several emails sent to notify you of the necessary forms, to be completed, before the appointment is cancelled. For questions please either call or message the office via our website.
All New patient appointments (in office or telehealth) are $105.00 and the payment is due the date of service.
