In the meantime, we offer clear out-of-pocket pricing and can provide you with a superbill (a detailed receipt) to send to your insurance for possible reimbursement. We’re always happy to answer any questions about costs, payment options, or insurance updates—just reach out!

Botox Re-Enrollment Telehealth Appointment (For Previous Patients of Brooke) 30 Min

This appointment is for previous patients of Brooke who have already been approved for Botox treatment and need to establish care at our current location. It includes a brief review of prior treatment history, confirmation of insurance authorization, and enrollment into our current office system.

Please Note: This appointment must be scheduled at least 4 weeks before your next Botox injection is due to allow sufficient time for prior authorization processing.

New Patient Appointment - In Office 60 Min

An in-depth evaluation to identify the root causes of migraines and headaches, while developing a customized medication plan that addresses both immediate relief and preventive care.

New Patient Appointment - Telehealth 60 Min

A virtual, in-depth evaluation to identify the root causes of headaches and migraines, with a customized medication plan designed to provide both immediate relief and long-term prevention.

Follow-Up Appointment - In Office 30 Min

Follow-up visit for established patients to manage chronic conditions, review labs, adjust medications, or discuss ongoing care. (Must have seen Brooke Phenicie, FNP-BC within the past year.)

Follow-Up Appointment - Telehealth 30 Min

Follow-up telehealth visit for established patients to manage chronic conditions, review lab results, adjust medications, or address ongoing care needs. (Must have seen Brooke Phenicie, FNP-BC within the past year.)

Botox for Chronic Migraine 30 Min

For patients approved for Botox treatment for Chronic Migraines. This therapy is designed to provide lasting relief for those experiencing 15 or more headache days per month and has been approved by their insurance provider or healthcare professional.

Botox for Cervical Dystonia 30 Min

This visit is for patients approved for Botox to treat Cervical Dystonia. Our experienced practitioner uses targeted injections to reduce muscle spasms, relieve neck pain, and improve head posture—providing long-term relief and a better quality of life

Cosmetic Botox/Xeomin/Dysport Treatment 30 Min

This visit is for patients seeking cosmetic treatment with Botox, Xeomin, or Dysport. Our experienced practitioner uses these neuromodulators to soften fine lines and wrinkles—most commonly in areas like the forehead, frown lines (11s), and crow’s feet—to create a smoother, refreshed appearance.

Pricing is based on the number of units used, which varies depending on your individual treatment goals and muscle activity. A personalized plan and cost estimate will be discussed during your appointment."

Trigger Point Injections for Migraine 15 Min

This targeted treatment helps relieve migraine pain by calming overactive nerves and muscles. Local anesthetic and/or corticosteroids are injected into trigger points to block pain, relax tension, and reduce inflammation.

Migraine Infusion Appointment 30 Min

IV infusion therapy for acute migraine relief, including hydration, anti-nausea, pain relief, and migraine-specific medications—administered in a calm, supportive outpatient setting.

Botox for TMJ/TMD Relief 30 Min

This quick Botox treatment targets the jaw muscles to help relieve pain, tension, and clenching associated with TMJ/TMD. Most patients experience noticeable relief within a few days. Price includes the actual Botox.
