• Completion of an appointment request does not guarantee establishment of care or provider acceptance.

• New patient appointments may require verification of insurance benefits and review of clinical appropriateness prior to confirmation.

• If you are experiencing a medical or psychiatric emergency, call 911 or go to the nearest emergency room immediately. Do not use online scheduling for urgent concerns.

• Telehealth appointments require the patient to be physically located in the State of Arizona at the time of the visit unless otherwise permitted by law.

• Please complete all intake paperwork prior to your scheduled appointment to avoid delays or cancellation.

• Missed appointments or late cancellations may be subject to a fee in accordance with office policy.

• Controlled substances are prescribed only when clinically appropriate and are not guaranteed at the initial evaluation.

• By proceeding with scheduling, you acknowledge that electronic communications and appointment reminders may be used in accordance with our privacy and communication policies.
