New Patient Scheduling Requirements
*** All new patients must request one of the Initial Psychiatric Evaluation appointment types before requesting any follow-up visit.
*** Follow-up appointments cannot be scheduled until after the initial evaluation is completed and care is formally established.
All appointment submissions are requests only. Each request is reviewed by the provider to determine clinical need, availability, and appropriateness for telepsychiatric care.
*** Intake paperwork must be completed at least 48 business hours before the scheduled evaluation. Failure to complete required forms on time may result in: Paperwork processing fees, and/or Release of the appointment slot to another patient.
