If this is your first visit, please select an Initial Evaluation.

If you’re an established patient, you may book an Established Patient Follow-up visit.

Federal insurance regulations prohibit me from accepting cash payments from patients who are enrolled in Medicare or Medicaid programs. If you are covered by either of these programs, I am not legally authorized to process payments outside of insurance, and I cannot provide services to you at Blue Zone Health and Wellness.
