Upon scheduling, we will e-mail you a questionnaire and confirm your appointment. There is no charge for consultations but there is a $50 no show fee. Please cancel your appointment with at least 24 hours notice if unable to attend. Please note that your BMI (Body Mass Index) must be ≥ 26 to qualify for medical weight loss.
