Important Booking Information

• Forms: Intake and consent forms must be submitted online at least 24 hours before your appointment. Appointments may be canceled if forms are not submitted on time.
   

• Insurance: In-Network with Mountain Health Co-Op, BlueCross BlueShield, Allegiance, Cigna, Pacific Source, Aetna, and First Choice Health. For other insurances, out-of-network but can provide a superbill for reimbursement. Always check eligibility directly with your insurance provider prior to the appointment. Our clinic does not check eligibility or benefits.
  
   

• Not Covered: Medicare, Tri-Care/Veteran, and Montana Medicaid do not cover naturopathic care.
   

• Payment: If out-of-network or no insurance, full payment is due at the appointment. Please call our office for self-pay pricing and more information.
