each box includes a sterile urine sample collection cup, flu test, covid test and urine tract infection test kit, FDA approved for at home use. Each box can be purchased and used prior to expiration dates and followed up with a telehealth visit with this clinic, for time sensitive, appropriate treatments. Each annual direct care subscriber household receives a kit for use with an active subscription. Additional visit charges for non-subscribers, will apply to review the results and discuss treatment options. Please note that outside clinics may not accept these tests or results, performed using testing outside of their own clinics. These are good until each test expires individually or is used with telehealth follow up with this clinic exclusively. Any expired tests or tests that are used in conjunction by a telehealth visit with this clinic, will be replaced, for subscribers only. No other replacements will be completed.
