Schedule this appointment if you would like to be seen for a FACE-TO-FACE at our Pop-Up Clinic in DeSoto, TX. During this visit, our provider will complete a comprehensive physical exam/office visit, order labs and/or medications as needed, and discuss your individualized plan of care to ensure your optimal health outcomes. Please bring your current medication bottles or an updated list of your current medications to this visit.
Note: There will be an additional fee for weight loss management visits.
NOTE: No Narcotics or Benzodiazepines will be prescribed.
Schedule this appointment if you are unable to reach your primary care provider and are in need of a temporary refill. NO NARCOTICS OR BENZODIAZEPINES will be prescribed. Must be able to show your current medication bottle for verification. Do not book for any medications that you are not currently taking.

This is a FOCUSED conversation where we review your health history, discuss your goals, and determine whether we're the right clinical fit for each other. Specific questions related to your current diagnosis, current treatment plan, medication use, nutrition, and/or overall wellness may also be addressed. To prepare for your visit, please have your questions readily available to ensure the time is utilized effectively.
If we're a match, we'll schedule your Initial Consultation from there.
NOTE: No new symptom evaluations, new diagnosis, new lab orders, or medication refills will be completed during this visit.
