Important Booking Information
Thank you for choosing Freedom Medical Center for your Health & Wellness needs. We are dedicated to providing you with the best care possible. Same day booking is not available online, but feel free to call us at 317-505-1747 as we occasionally have appointments available. Please take a moment to review the following information before booking your appointment:
1. Appointment Types:

We offer a range of services, including Medical Weight Loss consultations and IV Therapy sessions. Please select the appropriate appointment type when booking. In person appointments available and will be at office location in Fortville, IN. Services available throughout Indiana for telehealth.
2. Cancellation Policy:
We understand that plans can change. If you need to reschedule or cancel your appointment, please do so at least 24 hours in advance. Late cancellations or no-shows will be subject to a $50 cancellation fee. IV infusions are prepared prior to appointment, so any same day cancellations will be subject to 100% of infusion fee.
3. New Clients:
If you're a new client, please arrive 10 minutes before your scheduled appointment time to complete any necessary paperwork (unless completed prior to appointment). You will be required to provide identification at your first visit. ie. Drivers License.
You will be emailed New Patient Intake forms & Consents. Please complete these 24 hours prior to your appointment time for your provider to review, to ensure a timely visit.
4. Insurance and Payment:
We do not accept insurance, however we are happy to provide you with a superbill for you to provide to your insurance company, upon request. We DO accept HSA & FSA cards. Payment is required at the time of service.
5. Health & Safety:
Your health and safety are our top priorities. Please request a mask if you are feeling unwell at the time of your visit.

7. Children Policy:
We absolutely love seeing families and understand that sometimes kiddos need to tag along! As our space continues to grow and welcome more guests, we're also mindful of keeping it calm, safe, and focused for everyone. Because there are medications, needles, and other sensitive items throughout the facility, we kindly ask that children only come if they’re able to sit calmly and remain by your side without exploring or causing disruption.
