Medication pickup is available at 9603 Red Fox Drive in Mount Horeb during business hours. Patients must fill out their Refill Questionnaires and have a signed consent form on file. Not having either of these documents will lead to a delay in your prescription being filled.
We?re happy to offer after-hours appointments when possible! Please note that any after-hours bookings (such as weekends or evenings) must be requested no later than noon (12:00pm) the evening you are requesting the refill for except on Fridays. After-hour refill requests on Friday must be made no later than 10:00am that day. This ensures your treatment is ready and allows us to provide the same quality of care outside normal hours, and we cannot guarantee preparation for requests submitted after this timeframe.

Refills are delivered to Belmont every Thursday morning.
If you need something shipped to you, select this option. A flat fee of $35 is required to send your order and invoices must be paid before your package is sent out.
Refill Shipments must be scheduled no later than the end of the previous business day (Mondays). We only ship out on Tuesday mornings, so that means your appointment must be scheduled by the time we close on Monday.
If you are looking online to schedule a shipment, you will see that Tuesday morning is the only available appointment time. That is to help keep shipments running smoothly!
