There will be a 2.99% service charge for cards

$12 a unit 
Can be used on forehead lines, frown lines (11 lines) or crow's feet (or any combination of the 3) 
Injections typically last 3-4 months 
You can inject up to 20 units in the forehead lines, up to 20 units in the frown lines and up to 24 units in crow's feet (bilaterally) every 3 months=84 units
Come into the office after a week for touch ups if needed 
It is not recommended you do an area independently of the others

$159 per month with a reoccurring charge at the 1st of each month
