Menopause Bioidentical Hormone Replacement Package - recurring/monthly (Initial Visit - In Office) 45 Min
INITIAL VISIT - IN OFFICE/IN PERSON 
Package includes initial consultation, lab order/review and prescription for compounded hormone medication, if warranted. Menopause BHRT package is $250/month. Minimum of 6-month commitment. 10% discount for paying 6-months up front. DPC members receive 10% discount off Menopause BHRT package.

Includes all lab work and follow-ups. No copays, no surprise bills.
