Perimenopause Optimization Program - recurring/monthly (Initial Visit - Telehealth) 45 Min

INITIAL VISIT - TELEHEALTH
Package includes initial consultation, comprehensive hormone lab work, lab order/review The Mom NP Guided Program, and prescription for compounded hormone medication, if warranted. Perimenopause package is $275/month. Minimum of 4-month commitment. 10% discount for paying 4-months up front. DPC members receive 10% discount off Perimenopause package. Includes all lab work and follow-ups. No copays, no surprise bills.
