New patient visit is an initial consultation to establish care to review your acute and chronic medical history, medications, preventive care, and future plan of care.  

*PLEASE NOTE* - Axis Health will not prescribe chronic narcotics for patients who are not established active patients.

New patients establishing care via tele-health virtual visit must have in-person visit follow up within 14 days for complete physical assessment.
