This initial visit is for new patients enrolling in our Direct Primary Care weight management program. The visit is comprehensive and focused on understanding the medical, metabolic, and lifestyle factors that influence weight and overall health.
During this extended visit, we will:
  • Review your medical history, weight-related history, medications, and prior weight-loss efforts
  • Discuss nutrition patterns, physical activity, sleep, stress, and other lifestyle factors
  • Screen for cardiometabolic risk factors and contributing conditions
  • Perform a focused physical exam as appropriate
  • Review or order baseline laboratory testing
  • Discuss individualized treatment options, which may include nutrition strategies, behavior change, lifestyle interventions, and medication support when appropriate
  • Create a personalized, realistic plan focused on sustainable results and long-term health
This visit allows dedicated time to develop a thoughtful, individualized approach—without rushing—while setting clear expectations for follow-up, communication, and support within our Direct Primary Care model.

  • Provide preventive care, health optimization, and lifestyle guidance
  • Address new concerns that do not require an urgent visit

During this visit, we will:
  • Review your medical history, current medications, and prior hormone testing or treatment
  • Discuss symptoms such as fatigue, weight changes, sleep disruption, mood changes, libido concerns, and temperature intolerance
  • Review lifestyle factors including nutrition, physical activity, sleep, and stress
  • Screen for underlying metabolic, cardiovascular, or endocrine contributors
  • Perform a focused physical exam as appropriate
  • Review or order baseline laboratory testing
  • Discuss individualized treatment options, which may include lifestyle modification, targeted supplementation, and hormone therapy when clinically appropriate
  • Establish clear goals and outline follow-up and ongoing support within our Direct Primary Care model
This visit provides dedicated time for education, shared decision-making, and personalized care planning. Our approach emphasizes safety, evidence-based care, and long-term health optimization rather than quick fixes.
