Our process involves:
1. Scheduling a New Patient Appointment ($150 non-refundable deposit required)
2. Completing Recommended Lab Work
3. Scheduling a 60-minute Extended Lab Review
4. Developing a Personalized Treatment Plan with Your Provider
5. Periodic Check-ins with Your Provider and Lab Work as Needed

Please note. We are a cash-pay clinic and do not bill insurance. Payment is due at time of service. We will provide you with a superbill you can submit to insurance for possible reimbursement.

**Former patients are asked to schedule a 60-minute Extended Follow-Up to establish care at Radiant Health Collective.

This is a comprehensive initial consultation for adults designed to thoroughly review your health history, current concerns, and long-term goals. Your provider will take time to discuss recommendations, including lab work and personalized next steps to support your health.

IN PERSON. Select this appointment if you have fewer than 8 labs to review, or if you are checking in with your provider on your treatment plan, symptoms, and progress.

Select this appointment type for an injection visit or injection consultation with one of our providers. 
PHONE. Select this appointment type if you have been referred to our clinic for a Coordination of Care visit by a naturopathic doctor. This appointment establishes prescriptive rights with our ?nurse practitioners.
