To avoid appointment delays and or interruptions, please complete your new patient enrollment documents 24 hours prior to your appointment and if you are an established patient keep your insurance and contact information current.

Office/Home visits stop at 2pm.
If an office/home visit is scheduled after 2pm it will be cancelled.
We only accept traditional medicare insurance, no other insurance is accepted. All non-insured initial medical visits are 175 dollars, Speciality visits-Psych/Hormone/MCC visits are 275 dollars, medical follow up visits 100 dollars , Specialty visits follow up 150 and 75 for follow up virtual visit. We do have payment options call t inquire about details. Although we only accept traditional Medicare, we collect your insurance to use for lab orders and medication where applicable. If you do not provide your insurance information you may receive a bill for these services.
We do not have office hours, virtual visit or make house calls visits on the following holidays.
