After booking your appointment you will receive an email with a lab requisition. These must be completed at least a week prior to your scheduled appointment *keep this in mind when booking* if you have a menstrual cycle, these should be completed between days 19-21 of your cycle. ALL CONSENT FORMS AND PATIENT QUESTIONNAIRES MUST BE FILLED OUT 72 HOURS PRIOR TO ATTENDING THIS APPOINTMENT. 
If forms are not completed at least 72 hours prior to your appointment, we will have to discuss rescheduling your appointment as this will inhibit our ability to adequately prep your initial recommendations that will be discussed at your consultation.

A $100 deposit is required when booking your initial consultation. This deposit will be credited toward the cost of your consult ($250) at the time of your appointment.

Please note: The deposit is non-refundable for cancellations less than 48 hours prior to the appointment or no-shows.
