Appointment & Cancellation Policy

Proficient Health & Wellness is a cash based clinic and does not bill insurance.

A valid card on file is required to book any appointment.

Appointments must be cancelled or rescheduled at least 48 business hours in advance. Late cancellations, no shows, or arrivals over 15 minutes late may result in a fee charged to the card on file.

All required forms must be completed before your appointment or it will be rescheduled.

By booking, you authorize applicable charges per clinic policy.


Additional Form Completion

This add-on is for completion of additional paperwork related to a previously completed assessment or visit. Examples include additional employer forms, leave forms, return-to-work forms, disability forms, revised paperwork, or other documentation requiring provider completion.

This fee covers one additional form.

This add-on does NOT include new medical evaluations, FMLA evaluations, disability evaluations, return-to-work assessments, perioperative risk assessments, or post-operative assessments. Services requiring a new medical evaluation must be scheduled as a separate appointment.
Additional forms requiring extensive review or completion may result in additional fees.
Rush Processing prioritizes your paperwork or documentation request for completion within 48 business hours after all required information, forms, records, test results, and supporting documentation have been received.

This add-on may be used for eligible paperwork and documentation services, including medical forms, employer forms, return-to-work documentation, surgery-related paperwork, and other provider-completed documents.

Designed for patients who have recently undergone cosmetic surgery and would like a medical follow-up evaluation after returning home. This visit focuses on assessing your recovery progress, reviewing post-operative instructions, evaluating symptoms and concerns, and providing recovery education and guidance.

Includes:
? Review of operative and discharge instructions
? Assessment of recovery progress
? Vital signs evaluation
? Surgical site observation
? Review of post-operative symptoms and concerns
? Recovery education and recommendations

Important:This service is intended for recovery monitoring and education only. It does not include completion of FMLA paperwork, disability forms, employer forms, return-to-work documentation, or other medical paperwork. Patients requiring documentation or work-related forms should schedule a Post-Operative Recovery & Documentation Assessment.

This service does not include wound care, drain management, lymphatic massage, suture removal, prescription management, treatment of surgical complications, or emergency medical care. Patients experiencing urgent or concerning post-operative complications should contact their surgeon or seek appropriate emergency medical attention.
IMPORTANT: A non-refundable deposit is required at booking and covers required laboratory testing. Labs must be completed before your appointment. An EKG is required for all patients and is obtained through an outside provider at a separate cost. Failure to complete required forms, questionnaires, and labs before your appointment will result in rescheduling. Additional testing may be required and may result in additional fees.

Perioperative Risk Assessment for patients preparing for surgery. Assessment cannot be finalized until all required testing has been completed and reviewed.
Designed for patients who have recently undergone cosmetic surgery and require medical documentation related to their recovery.

This visit includes a post-operative recovery assessment, review of surgical records, evaluation of functional limitations, and completion of one employer, disability, leave, or recovery-related documentation packet when appropriate.

Includes:
? Review of operative report and discharge instructions
? Post-operative recovery assessment
? Evaluation of recovery progress and functional limitations
?Completion of one employer, disability, leave, or recovery-related form
? Work restriction recommendations when clinically appropriate

Requirements:Patients must provide a copy of their operative report and discharge instructions prior to the appointment.

Important:Documentation, work restrictions, and recommendations are based on clinical findings, recovery status, and available surgical records. Approval of leave requests, disability benefits, work restrictions, or other documentation cannot be guaranteed.

This visit does not include clearance to return to work, removal of work restrictions, or fitness-for-duty determinations. Patients requiring return-to-work clearance or modification/removal of existing work restrictions must schedule a separate Return-to-Work Assessment and applicable fees will apply.

Additional forms beyond the first form included with this visit may require an Additional Form Completion fee.
Designed for patients recovering from cosmetic surgery who need an evaluation before returning to work.
This visit focuses on assessing recovery progress, physical limitations, mobility, and ability to safely perform job duties following surgery. Based on clinical findings and available surgical documentation, return-to-work recommendations and work restrictions may be provided when appropriate.

  • Assessment of mobility and functional status
  • Evaluation of work-related limitations
  • Review of surgeon recommendations when available
  • Return-to-work documentation
  • Work restriction recommendations when appropriate

  • Lifting restrictions

  • Prolonged sitting
  • Bending, twisting, and reaching
  • Driving restrictions
  • Physical activity limitations
