Office Policies for your visit: PLEASE READ:
1.All forms must be complete at least 48 hours prior to booking, if you need assistance with this please call 586-450-8551, we are happy to help and/or mail a packet prior to appointment. Our practitioners will be evaluating your situation prior to your visit therefore ensuring these are completed prior will ensure they can become familiar with you and your case before your face to face. This will also allow more face to face time with your practitioner at your initial appointment
2.Appointment times are strictly enforced. If you are running late you must call prior to your appointment time to check if we are able to accommodate your visit still. If you are greater than 10 minutes late for your appointment, you will be rescheduled and charged a $75 late fee. If we are able to accommodate you if you are late, you will only receive the original time slot that was provided at scheduling. (i.e. Appointment time was scheduled from 10am to 10:30am, you arrive and are seen at 10:15.You will be responsible to pay for the full 30 minutes but only be given 15 minutes until 10:30am).
3.Confirming Your Appointment: It is your responsibility to confirm your appointment through your online portal at least 24 hours prior to your scheduled appointment. Our office will contact you 1-2 days before your appointment to confirm, however if you miss this call you will need to call us back or log in to mark "confirm" on your scheduled date and time. If you are not confirmed in our system the morning of your scheduled appointment your visit will be rescheduled and a $75 fee will be charged.
4.Medical Records (First Visit): If you have any specialty testing, bloodwork, Images (CT scans, X-rays, PET scans) within the past 3 years please upload and/or bring to appointment. If you do not have access to these records, please complete the medical release and contact our office (through portal messenger and/or phone), and provide us with the medical facilities, physicians, and contact of places you had testing. Please allow 72 hours for us to obtain your medical records if we are having to request from outside facility.
5.Medications, (OTC and Prescription), and Supplements/Vitamins/Herbal Remedies: Please bring all current supplements and medications or a comprehensive list medication name (if supplement OTC include brand), dosage, frequency of taking, and reason for taking.
6.Medical Insurance: As stated in the original forms we do not accept medical insurance due to the limitations in services this provides. If you choose to proceed with asking your insurance company for reimbursement of services we are happy to provide you a detailed list of services you received, diagnosis codes, and pricing breakdown. We will not be responsible for any services, denials, or bills received from your insurance company. You understand it is your responsibility to call and check what is covered by your insurance company. If you are provided a lab order for bloodwork at your visit with us, you are responsible to call your insurance company prior to obtaining the ordered test to ensure coverage. If your insurance company does not cover testing ordered by any of our practitioners we are able to order them through a cash-based discounted lab, which would only be available prior to testing. You understand if you proceed with testing and your insurance company denies to pay for testing, you are unable to proceed with the cash based discounted option and you are responsible for the payment needed to the diagnostic center in which insurance did not cover.
7. We encourage you to bring a support person with you to your first visit s a lot of information will be given and a second person can be very helpful. Our medical model differs from a conventional approach, we will explore many processes in the body specific to you and this is usually identifying multiple body systems that are interconnected. Your first appointment can be overwhelming if you have never experienced a true functional medicine consultation. Our goal is to identify your primary concerns, and then work upstream to identify all the interconnected links that are responsible for these concerns. From there we can then work systematically to address in an objective way using pathophysiology and personalized medicine.
