This service if for ESTABLISHED PATIENTS desiring a wellness exam for their child who is presenting for their
4mo, 6mo or 9mo wellness exam. 

This service is for NEW PATIENTS for a Well Child Check Up for all ages 12mo+.

This service is for ESTABLISHED PATIENTS for a Well Child Check Up for all ages 12mo+. 

Traditional schedule newborn (5-7 days), 2wks, 1mo, 2mo, 4mo, 6mo, 9mo, 12mo, 15mo, 18mo, 24mo, 30mo, 3yrs, annually

*All testing, if any, will be an additional cost and discussed ?before collection. 

A child who is ill with various conditions such as:

- new onset rash* 

- fever

- new onset headache

- sore throat

- cough, congestion

- new onset fatigue

- Ear pain/drainage 

- Eye drainage/redness*

- weight/growth checks (not "sick")

Required for most school-aged kids who plan to participate in sports, marching band, sport leagues, certain camps, etc.
