DHD Assessment Consultation
This consultation focuses on reviewing medical history and current concerns that may indicate Attention-Deficit/Hyperactivity Disorder (ADHD).

The ADHD Assessment Consultation is part of a structured process for screening, diagnosing, and treating ADHD. This service is offered as a bundled evaluation priced at $1,000 for new patients who have not established care and $800 for established patients. Full payment is required at the time of scheduling the ADHD Assessment Consultation.
The ADHD Evaluation Bundle includes:

1. Assessment Consultation
   • 60 minutes for new patients
   • 45 minutes for established patients
2. Barkley Assessment of Executive Functioning and Cognitive Deficits (completed at home)
3. QbCheck computerized ADHD assessment (completed at home)
4. Results and Treatment Planning Visit (75 minutes). During this visit, we will review all assessment results, determine whether diagnostic criteria for ADHD are met, and develop a comprehensive treatment plan using principles of Functional Medicine and Integrative Psychiatry.

Important: All questionnaires and the QbCheck assessment must be completed at least 7 days before the results visit to allow adequate time for review.

PANS/PANDAS Diagnostic and Treatment Visit

This visit focuses on reviewing laboratory results and clinical history to determine whether diagnostic criteria for PANS or PANDAS are met. During this visit, we will confirm the diagnosis when appropriate and develop a comprehensive treatment plan using principles of Functional Medicine and Integrative Psychiatry.
