1. Get Your Bloodwork!
Bloodwork within the past 6 months is required before your consult with a Health Maverick.

- If you already have recent bloodwork, you can upload it to the Optimantra Patient Portal.
2. Pick Your Time Slot
Lock in your consult with a Health Maverick by clicking the Proceed button at the bottom of the page.
3. Register as a Client
Register as a new client with just a few basics.
- Already part of the H4M crew? Just use the "Already A Client?" button and you're good to go!
4. Get Confirmation
Get instant confirmation and an email with all the details for your upcoming appointment.
Pro tips for a smooth start:
- • Don't forget to upload your bloodwork!
- • On the next page, check the "Opt-in to text reminders" box so you never miss an update.
- • Before your appointment, complete the consent form you'll get in your confirmation email.
