provider visit: Includes consultation, physical exam and lab work* (must have labs completed, see pre-consultation lab work) with order of testosterone, if applicable. Can be telehealth or in-person

Follow up visit:  includes consult and labs if needed and testosterone/supplies order when needed.  Visits May be telemed or in house visit, required by state and federal law every 90 days as is controlled substance

Follow up labs draw an additional $70 if needed.

Visit includes follow-up assessment with provider, labs if needed, alterations in therapy if needed, education/information

The telehealth acute visit is cash pay and for those occasions you feel too sick to come into the office and physical exam is not needed.  If we determine that physical exam is needed, we may ask you to come to the office for further examination/labs or other services needed face to face.
