Primary Care with Candyce Poteet, MSN, APRN

  • Wellness visits & preventive care
  • Well-woman exams & annual physicals
  • Sick visits (colds, flu, infections, etc.)
  • Work, school, or sports physicals


Functional Medicine with Lisa Fournace, DNP, APRN & British Bates, MSN, APRN

  • Deeper dive into root causes of chronic issues
  • Gut health, thyroid, autoimmune conditions
  • Diabetes & metabolic health
  • Hormone replacement therapy (HRT) & optimization
     


Payment & Policies

  • Cash pay only – no insurance billing for visits
  • FSA/HSA accepted – insurance may cover labs (except in-office rapid tests)
  • Deposits required – non-refundable, may be applied to rescheduled visits
  • No refunds – full payments and deposits are non-refundable
  • Late cancels/missed visits – <24 business hours = $100 fee; no-shows = full appointment charge



Before Your Visit

  • Check your email (including spam) for required forms
  • New patients: you’ll receive a welcome email to set up your Optimantra portal
  • All paperwork & consents must be completed before your visit
  • Adults only (18+) – no pediatric visits
