This appointment is for established patients only. A 30 minute visit conducted through a private, HIPPA compliant telehealth link. If you are planning to review your lab results with Susan, please confirm with the office that we have received the results.

Health Coach - 30 Minute 30 Min

30 Minute Health Coach Appointment

This appointment can be scheduled by current patients that are already being treated by Susan. If you are interested in seeing a health coach and are not a current patient, please schedule a New Patient Visit.

Discovery Call 15 Min

Discovery Call

A functional medicine approach requires you to commit time, energy, and resources. Before doing that, we understand that you want to make sure we are a good fit for you. That is why we are now offering 15-minute discovery calls. This will give me an opportunity to explain the functional medicine approach to you, you can ask me questions, and we can decide together if we would be a good fit to work together. All proceeds from our Discovery calls are donated to the Henry County Pregnancy Care Center.

2026 New Patient- 99204 60 Min

New Patient Visit $399.00

One-hour initial intake visits for new patients include face to face time with Susan by video appointment> during this visit she will LISTEN to all your concerns, ask some difficult and detailed questions. This investigative approach will help her get to the root causes of your concerns. During this visit she will determine what further testing needs to be done before your treatment plan follow-up appointment. We ask that you log into your new patient portal and fill out the medical history and consent forms before your scheduled appointment time.

Member Phone Call 10 Min

For current members only. This is a ten minute phone call that can be used between visits with Susan. It can be used 3x/calendar year per member.
