Welcome to MMHS Online Appointment Bookings. The provider is licensed in AZ, CO, GA, FL, MD, and NY. If you are a new patient, please select New Patient Appointment and complete all the necessary intake forms so that we can confirm your appointment promptly. If you are an existing patient, please select the Follow-Up option and a time slot, and we will confirm your appointment within 24 hours. Note that a payment card is required, but you will not be charged at the time of booking. 

MMHS values each patient's time and needs for care. Thus, if you book an appointment, that time slot is reserved only for you. We do not take walk-ins. The provider values your time. So, if you cannot keep your appointment for whatever reason, please cancel your appointment before 24 hours of your scheduled time to allow another patient to fill that time slot. All appointments canceled or rescheduled within 24 hours of the scheduled time will incur no-show/late cancellation charges per practice policy. We are excited to serve you soon.
