After you schedule your New Patient Consultation, you will receive a confirmation email which will also include directions to the office, as well as health history and consent documents. Please complete those BEFORE your appointment.

In order to secure your spot, online appointments may be require a credit card on file. You will not be charged for your visit ahead of time.
Missed appointment and late cancellation policy:
Please call at least 24 hours in advance to cancel or change your appointment. This will allow someone else to take that spot. Late cancellation and missed appointments incur a $50 fee, unless the cancellation is due to a true emergency or bad weather ( ex: mud slide, flood, road closure).
