Please review the following before booking your appointment:

• We are a cash-pay practice and do not bill insurance, however HSA/FSA is accepted. Payment is due at time of service. We are happy to provide a superbill that you may submit to your insurance for possible out-of-network reimbursement. Insurance may be used to cover lab fees whenever possible according to your specific policy coverage. 

• A card hold is required for booking. A $50 cancellation fee will be charged for no-shows or if appointment is cancelled less than 24 hours before scheduled appointment time. No cancellation fee will be applied if the appointment is rescheduled before the start of the original scheduled appointment time. 

• All New Patient appointments will be in person. We do not offer telehealth visits for initial consultations at this time. You may discuss with Provider the option to have follow-up appointments via telehealth. 

• You’ll receive a confirmation email with intake forms after booking. These must be completed at least 24 hours before your visit.

A comprehensive evaluation for women of all ages experiencing hormone-related symptoms. This visit includes an in-depth review of your health history, symptoms, lifestyle, and goals. Common concerns include fatigue, sleep disturbances, weight gain, PMS, irregular cycles, perimenopause, menopause, thyroid dysfunction, PCOS, and more.

A focused visit to review lab results, monitor progress, and adjust your treatment plan as needed. We’ll assess how you’re responding to lifestyle changes, supplements, or therapies, and address any new concerns. Follow-ups help ensure you’re supported through every phase of your hormone and metabolic health journey. This visit will take place in our office and is recommended for your first follow-up visit.

This appointment is designed for men looking to better understand their hormone health. We’ll review your symptoms, lifestyle, and medical history in depth. If indicated, lab testing will be ordered to evaluate key markers like testosterone, DHEA, cortisol, thyroid function, cholesterol and other related health biomarkers. This visit may be the first step toward treatment options such as lifestyle support, nutritional optimization, or medically supervised testosterone replacement therapy (TRT), if appropriate.

During this appointment, we’ll review your most recent lab results, assess your progress, and adjust your treatment plan as needed. Ideal for reviewing initial labs after your first visit, ongoing follow-up for testosterone replacement therapy (TRT), or monitoring hormone health and symptoms for men not on TRT. This visit will take place in our office and is recommended for your first follow-up visit.

Trigger point injections are a therapeutic treatment designed to relieve tight bands of muscle tissue—commonly known as “knots”—that can cause localized pain, stiffness, and referred discomfort in other areas of the body. These trigger points often develop due to injury, stress, overuse, or postural strain. Your provider will gently inject a small amount of solution (typically lidocaine, or other non-steroidal options) directly into the trigger point to help relax the muscle, reduce inflammation, and restore normal movement patterns.  Initial treatments are best done in a series of three separate sessions, about one per week for three consecutive weeks. Subsequent treatments may be done on an as needed basis. Discount packages available. *Please note area of concern in the Comments box when scheduling.

This follow up appointment is for established patients who have already completed their initial consultation and food sensitivity testing. During this visit, we will review your results in detail and discuss personalized dietary recommendations and next steps.
This service includes is for patients interested in food sensitivity testing to better understand how certain foods may be contributing to digestive symptoms, inflammation or other chronic concerns. This includes a 30-minute initial consultation and 30-minute follow-up visit. We'll review your health history, discuss your symptoms and determine whether food seems to be testing is appropriate for you. The follow-up visit will review your results in detail and discuss personalized dietary recommendations based on your results, along with practical elimination and reintroduction strategies.

Testing is completed at home and processed through a third-party lab. This consultation fee covers both consultations and professional interpretation. The lab fee is paid separately and directly to the lab at time of ordering. Patients are welcome to inquire about current lab pricing prior to testing. HSA/FSA funds may be used for the lab cost.
