Welcome to Lume Wellness – We’re so glad you’re here!
Please review the following before booking your appointment:
• We are a cash-pay practice and do not bill insurance, however HSA/FSA is accepted. Payment is due at time of service. We are happy to provide a superbill that you may submit to your insurance for possible out-of-network remibursement.
• A card hold is required for booking. A $50 cancellation fee will be charged for no-shows or if appointment is cancelled less than 24 hours before scheduled appointment time. No cancellation fee will be applied if the appointment is rescheduled before the start of the original scheduled appointment time.
• All New Patient appointments will be in person. We do not offer telehealth visits for initial consultations at this time. You may discuss with Provider the option to have follow-up appointments via telehealth.
• You’ll receive a confirmation email with intake forms after booking. These must be completed at least 24 hours before your visit.
• If you have labs that you'd like your Provider to review before your appointment, please email them to info@lumewellnessaz.com.
